I think this is not bug, because most likely that flare is found from NR/WH footlockers.

I think I obtained it through the tanker quest, in the final room with random items. This flare was found among the items.

Actually, I remember clearly now: It was found in the map the is entered through the stairs, in the top right room. There was also a superior 10mm pistol with it, it was beyond repair.
