This shows a "dead hex" at the player's bunker base. It's a hex that appears as normal ground hex in the wide open (shown here in red, next to the guy in power armor), but for some reason cannot be accessed:

The meeting table inside the bunker's conference room has a weird corner. If you stand at the end, like this guy in power armor, your head will be missing - covered up by table. Move 1 hex away and your head comes back:
