What places i need to discover for Explorer perk?

Hello there. I was wondering what exactly i need to be able to take Explorer take from that ghoul inside NCR station.
This is my World Map Navigate.

And World Map (With red squares i marked places i wasn't able to find/visit.)

Have you check all train stations?

Edit: You get Scavengers location from guy at Adytum.

The combat armored guy at the entrance.

I don't think Raiders is needed, but you can get to Sierra by talking to a hermit in encounters around it.
Toxic Caves you get from talking to a trapper somewhere in Klamath if i recall right.

Yeah, boneyard, ncr, san francisco and near vault city (i was doing that crazy quest from doc in vault city).

I'm going to check those u told me, if it won't work i will edit this post.

Okay, well here i'am with Toxic Caves, Scavengers, and Sierra Army Depot location on my map.
This ghoul Mantis still dont want to give me this perk. What else should i do?

That's my crafter/barter, so i gave it 10 int for best SP gaining. And i have 166% outdoorsman

I didn't visit The Glow and still got Explorer.

I checked train stations again. (Vault City, NCR, Boneyard, San Francisco)
Ghoul Mantis don't want to give me Explorer Perk anyway.
I hav all locations on my map except
- Raiders
- Vault 13
- Military Camp

afaik, you need only towns. And im not talking about visiting, just uncovering location

The perk cannot be gained unless the player has the location of all train stations, all towns and cities, and the Glow marked on the world map.

I have 166%. Is there possibility that if i have more than 150% i can't take that perk? D:
