I have to tell I disagree with most of your suggestions (with my two weeks of experience).
I'm not talking in the name of PvPers nor TC fighters.
I'm talking in the name of players who enjoy the feeling of Fallout and playing for fun without thinking the whole map is a huge pvp arena.
Followers, companions:
I think companion is a must when you level up at the beginning. I could hardly wait to reach lvl12 to get a companion who helps me in the wasteland, because dying in every 5 minutes because of PKers or in an encounter with 5 npcs when you have ~100 hp and shitty beginner guns, its not fun. My companion really saved my ass many times. It's not a solution to remove them completely.
Companions, mercs, slaves in TC _is_ an issue that needs to be solved.
Crafting:
It's easy for the mining/crafting abusers, yes, when you have private mines for all of your 10 alts with slaves.
I only have one and it's enough to make life easier without getting killed in unguarded mines. So it has its price.
A fee for mining only makes it hard for beginners to craft their first things. For experienced players it will be nothing and I imagine that everyone's mining alts will be there in the mines all day, played by bots and accompanied by a number of brahmins/followers.
I think it's the mining/crafting bots that needs to be regulated.
Rare items. Not everyone has a bunch of Brotherhood/Enclave armors or PAs. They are mainly accessible to TC fighters. What about the others?
Footlocker/cave searching is fun if its not abused with bots. Most of us got killed by PKers there btw.
I agree with your thoughts on reputation and rewards though.
