i was doing repeatable job from junktown called "Do i smell a rat" and 2 bugs happened
- once location spawned on top of Hub and i was not able to enter it

- after doing it 47th time on my main char, it didnt register that i killed all molerats in location and i couldnt finish it, i checked whole location to make sure there arent any left
later same bug happened on my alt after doing it like 30 times, i am not sure why its happening sometime and other time not, maybe some monsters are behind grid, hard to say
edit : happens with other, similar type of quests too

i killed all the rats and i cannot finish the quest... didnt give notification that i am done

Yeah it's being caused by some inaccessible rats still present in the map.

I don't think it was. Can't you just look into the code?

Was this fixed I can't remember .....
Did it few hundereds times since reducing amount of molerats spawning and i didnt had any problem with it registering killing of all rats anymore but i think it still can spawn on top of hub or necropolis. I didnt play last week but there wasnt any changelog so i guess that bug is still there unless there was some more ninja changes.
